If you're living with a chronic health condition, you may be eligible to access chiropractic care at Whitehorse Family Chiropractic through a Medicare Chronic Disease Management (CDM) Plan — at no cost to you.


What Is a Care Plan?


A Chronic Disease Management (CDM) Plan — also known as a Care Plan — is a Medicare initiative designed to support patients who have long-term or ongoing health conditions. Through this program, your GP can refer you to Whitehorse Family Chiropractic as part of a structured, coordinated approach to managing your health.


Who May Be Eligible?


You may be eligible for a Care Plan if you have a chronic condition that has lasted, or is expected to last, six months or more. Conditions that may be covered include:

  • Back or neck pain

  • Arthritis or joint conditions

  • Osteoporosis

  • Chronic headaches or migraines

  • Other ongoing musculoskeletal concerns


Your GP will assess your eligibility and prepare your Care Plan.


What Does a Care Plan Include?


A Care Plan provides:

  • Up to 5 chiropractic visits per calendar year under Medicare

  • A structured, coordinated approach to managing your condition

  • Ongoing communication between your GP and our team to support your care


Fees & Medicare Rebates


At Whitehorse Family Chiropractic, all Care Plan visits are bulk billed — meaning there is no out-of-pocket cost to you for these appointments.


How to Get Started


  1. Visit your GP and request a Chronic Disease Management Plan

  2. Obtain a referral — your GP will refer you to Whitehorse Family Chiropractic

  3. Book your appointment and bring your referral to your visit
